Enrolment Documentation:

 

To confirm enrolment, please submit the below completed documentation to the school office:
 

1. Enrolment form

2. Immunisation History Statement - for children under seven who are registered with Medicare, this statement can be obtained by calling Medicare on 1800 653 809 or by lodging an online request at the Australian Childhood Immunisation Register

3. Evidence of birth date - a copy of your child's Birth Certificate and, if available, a copy of Passport

4. International enrolments - a copy of your child's birth date and a copy of your child's Passport

5. Copy of Visa status - (if applicable).

PLEASE NOTE:

• Your child must turn five years of age by April 30 in the year of school entry.

• the originals of these documents must be sighted and photocopied at the school office.
